In my years in the improvement of quality in manufacturing, service and health care I see many different quality programs have involved met: Total Quality Management, Six Sigma, Plan-Do-Check-Act, Lean, Baldrige and Lean Six Sigma, the current favorite -as it seems. Which program is the best in health care? Are there really big differences between them? Let me answer that question with an illustration comparing Total Quality Management and Six Sigma.

Six Sigma and Total Quality Management (TQM) in health are very different in their approach to quality, even if they are complementary. Six Sigma focuses on reducing variability in the normal process, whereas TQM emphasizes a state of mind.
